Before continuing to apply for a service dog, please confirm you have the following documentation ready to upload.
These items are required to complete the application process and progress on the application cannot be saved.

Autobiography (In a separate document, please describe your life prior to your disability and how your disability affects your life now. Please describe how a dog would help.)

A recent photo of yourself

Professional Reference Report completed by your physician (you may print the form here, or have them fill out the form found online here)

DD214 (Veterans Only)

Letter of approval from facility or commander (if active duty military or applying for a facility dog)

Letter verifying service-related diagnosis of a disability (if applicable)

$100 non-refundable application fee (waived for veterans and active duty military)
